MAINTAINING YOUR NHS PLACE AT THIS PRACTICE
- Attending your appointment
- Completing your treatment
- Paying your NHS charges when requested
- Treating staff with the respect they deserve
Demand for NHS care is high, therefore it is reasonable to expect our patients to honour the above conditions in order to stay on our active list.
Registration with an NHS dentist ended in April 2006 so you are only registered with this Practice for the duration of a course of treatment. We encourage our patients to make their next routine appointment before leaving the Practice.
We have an active list of patients which is restricted due to our fixed NHS contract.
To help you to maintain a healthy mouth, all treatment must be completed and preventive advice followed. You will lose your NHS place if you regularly cancel or fail to attend your appointments.
An emergency is defined by the NHS as a situation requiring rapid access within 24 hours. This does not include sensitivity due to a lost filling or crown.
Thank you for your co-operation.
